Think of a line drawn without ever lifting the pen: from the first point to the last there is no cut, no gap, no pause where it stops and starts again. Everything else here is that same no-break picture applied to time, to a surface, or to a fixed term. One honest note about this entry: all four meanings listed are really uses of the adjective 'continuous'. 'Continuously' is its adverb form and carries the time meaning, but 'continuous assessment' and the grammar term belong to 'continuous' itself, not to 'continuously'.
