You can think of it as digging a living plant out of the soil it has been growing in, carrying it somewhere else, and settling its roots into new ground — the same living thing, but now rooted in a different place.
From Late Latin transplantare = trans- (across) + plantare (to plant): literally 'to plant across', i.e. to lift something rooted and re-root it elsewhere. Middle English took it in through Old French transplanter.
