Picture something pushing outward from the inside. The skin over an ankle after a bad landing is the clearest case: the surface is still the same skin, but there is now more underneath it than there was this morning, and it presses out until you can see the shape change. Every use of this word keeps that push. Sometimes the word names the raised place you can point at. Sometimes it names the state of being pushed out.
