Picture a supply window with a line of people in front of it. Everyone walks past and is handed the same thing: the same boots, the same coat. Nobody is asked which one they would like. Standard is the kind laid down by the rules, and issue is the handing out. The whole word is that picture: the plain version that comes to you because it comes to everybody.
