Picture a slot where an action was supposed to go, and nothing went into it: the payment was not made, the player did not turn up, no one chose an option. The word names that empty slot — and then, in a second step, whatever fills the gap when nobody acts. That is why one word covers 'failing to pay a loan' and 'the setting your phone came with': in both, the thing happened because nothing was done.
