One way to hold it: two flat surfaces meeting hard and fast, and the sharp sound that comes out. Hands are the usual pair, and doing it over and over is applause. Because the movement is so sudden, English also uses the word for any quick firm slap of a hand, and for one loud crack of thunder. The slang name for an illness is not part of this picture — it is a different word.
