You can hold mad as a mind that has slipped its normal track — reason gone, control gone, the inner steering wheel no longer answering. From that one picture everything fans out: a mind tipped off course becomes 'mentally ill' (the oldest use), then 'crazy / very foolish', then the heat of anger, then wild speed, then a love so strong it feels out of control. The thread running through all of them is: not held in by normal limits.
