Think of a voice pushed all the way out: something inside you — excitement, anger, fear, or just the distance between you and the other person — makes the normal speaking volume useless, so the sound comes out big. A yell is not careful speech. It is the voice at full force, and it usually tells the listener something about your state before the words even land.
