Utterance can be pictured as the moment a thought leaves the mouth. It was inside; now it is out in the air where other people can hear it. From that one moment the word looks two ways: at the leaving itself, and at the thing that left.

The word is utter (to say) plus the ending -ance, which turns an action into a noun; Middle English wrote it utteraunce. An ending like that naturally names both the act and its result, which is exactly the two-way split the word still has.
