Picture a shirt that has not been ripped once but worn and torn a hundred times, until it hangs off a person in loose strips that move in the wind. Two things are true of that shirt at the same time: it cannot do its job any more, and it is all still there for everyone to see. Keep both halves. The cloth gives you the plain noun and the phrase about clothes; the same picture laid over a plan or a person's good name gives you the meaning you will meet most often in the news.
