Picture the moment the smiling stops: whatever is being said or done now carries weight, so it is not to be laughed off. That weight is what the adverb adds. Point it at a manner and you get 'not joking'; point it at a degree and you get 'badly, to a worrying extent'; put it in front of a sentence and it warns the listener that the joking part is over.
