Start with the weather: thick white cloud sitting on the ground, so you can only see a few steps ahead and everything further away turns into a vague shape. `foggy` is that state of not-being-able-to-see-through. Move the same white cloud inside a person's head and you get the other uses — a mind that cannot see its own thoughts clearly.
