The letters lean, run together and spill over the line, and they land wherever paper happens to be — the back of a receipt, the corner of a , a wall. Both uses of scrawl come from that one hand: the act of writing that way, and the marks left behind once the hand has gone.

The source is not certain. Wiktionary gives Middle English scraulen, to spread out one's limbs or sprawl, itself an alteration of sprawl or crawl, and offers scrall, a contraction of scrabble, as an alternative. Both guesses point at limbs going everywhere, which suits letters that will not stay on the line — but this is offered as a possibility, not a fact.
