You can picture 'oral' as pointing straight at the mouth. Everything the word covers passes through the mouth: things spoken by it, things taken into it, tests answered out loud with it.
From Latin os (stem or-) 'mouth', via oralis 'of the mouth'. The mouth is the literal core; 'spoken' and 'taken by mouth' and 'a spoken exam' are all just what happens at that one opening.
