Built from formula + -ate: formula is a fixed, set form of words or a set recipe, so to formulate is literally to put something into a definite form.
這個字是 formula（公式、定式）加上動詞字尾 -ate 造出來的。formula 指的就是一套固定下來、可以照著走的形式——化學式、公式、配方都是。所以 formulate 字面上就是「把某個東西做成一個 formula」：把散的變成定的，把想的變成可以照著用的。這正好對上現代兩個用法都在做的事。
