Two things come with the picture. First, there has to be a group — nothing is uppermost on its own. Second, the top of a stack is what you reach first and see first. Move the stack inside a person's head, and the same picture gives you the worry that sits on top of all the other worries, and the concern that outranks everything else in a decision.

From Middle English upper-moste; by surface analysis upper plus the ending -most, the ending English uses for the furthest position in a direction (innermost, outermost, foremost). So the word is built to mean 'the one furthest up', which is exactly the picture above.
從中古英文 upper-moste 來，拆開看就是 upper（比較上面的）加上字尾 -most。-most 在英文裡專門表示某個方向上最遠的位置：innermost（最裡面）、outermost（最外面）、foremost（最前面）。所以這個字從構造上就是「往上走到最遠的那一個」，跟上面的畫面完全吻合。
