Think of someone going under — under the water, or under an illness — and something or someone reaching in and pulling them back before it is too late. Both meanings hold that one picture. The adjective names the thing that does the pulling back: the operation, the drug, the machine. The noun names the skill of doing it in water, learned on purpose and until it works.
