You can picture the place some religions describe waiting after death for bad people — a pit of fire, pain, and no escape. That single image of 'the worst place there is, full of suffering' is the seed for everything else. First it stretches to any time or place that feels like that pit: a job, a journey, an illness becomes 'hell.' Then, because hell is the most extreme, most forbidden word people reach for, it turns into pure heat for language: dropped into questions and before nouns to crank them up (what the hell, scared the hell out of me, one hell of a day), and shouted on its own when you're angry. A whole crowd of fixed phrases (give someone hell, raise hell, come hell or high water, when hell freezes over) all borrow the same picture — maximum trouble, maximum extreme.
