Picture someone asking: how is this person doing? — health, state of mind and living conditions taken together. There is usually a second person in the picture too: a parent, a teacher, an employer, a government, someone with a duty to check that they are all right. That second figure is what lets the same word slide from how a person is doing to the systems and money set up to keep them doing all right.
