One way to hold this word: your body is not working right — you're ill, off, . From that one bodily 'something's wrong inside me' picture, the word spreads two ways: outward to the physical reaction of an upset stomach (feeling like you'll vomit, actually vomiting), and figuratively to a mind or feeling that is '' — disgust, being fed-up, worry, even cruelty. (Recent slang flips it: 'sick' = so good it's almost too much.)
