Picture the very first big stone going into the ground before a building exists. Someone important sets it in place, the date is cut into its face, and then everything else is built on top of it. It stays at the bottom, out of sight, holding up what came later. Both uses of the phrase can be thought of as that one stone: the thing that went in first, and that the rest now stands on.
