A madwoman is a woman people call mad — in older English, that meant her mind was ill, or that she behaved in a wild way others could not understand. You meet the word in stories and plays more than in kind talk today: the madwoman in the attic is a famous picture in books, a woman hidden away. Though actually, it is a hard, old-fashioned word. Doctors do not use it, and it can sound cruel. In American English, mad often means angry; madwoman does not mean an angry woman. It keeps the older sense of out of her mind. If you need a careful word for illness of the mind, this is not it. The male pair is madman; both are built the same way.
