One way to picture johnny: the very common first name John, worn down into an everyday noun. In older British usage it simply meant 'a man, that fellow'. The other use — British slang for a condom — is a separate slang development, and the payload gives no line from 'a man' to it, so it is best learned as its own item rather than as a picture growing out of the name.
