Picture someone who has dropped out of ordinary life for a while: the bed is unmade at two in the afternoon, work is on hold, someone else is doing the shopping. Illness can be thought of as that whole state of not-being-well — the period a person is in, rather than the or the damage itself. That is why English says a long illness, a serious illness, off work with an illness: it is measured in time and in how much of your life it takes over.
