You can think of earshot as a circle drawn around a speaking voice. Inside the circle the words arrive; outside it they do not. English almost never talks about the circle itself, or about how wide it is. It only says which side of the line a person is standing on — and that is why the word shows up in just two shapes, within earshot and out of earshot.

The dictionary records the word as ear + shot. One way to read that: shot carries the feel of how far something sent out can reach, so a voice gets the same kind of range, and the ear is what it has to reach.
