Worse is simply 'bad, one step down' — the comparative of bad. Picture a staircase going downward: each step is more unpleasant, more harmful, lower in quality than the one above. That single move — taking bad and pushing it further down — runs through every use: a worse situation, worse health, doing something worse, the prospect of even worse to come. The adjective, the adverb (more badly), and the noun (something more unpleasant) are the same downward step seen through three windows.
