Vitals is a plural noun with two uses that are close but not the same.
The older, everyday one: your vitals are the organs you cannot live without — heart, lungs, liver, brain. It usually shows up when someone is hurt: 'the bullet missed his vitals', 'she was kicked in the vitals'. That second one is a polite, slightly old-fashioned way of saying the groin, and men especially will use it as a half-joke. This use has an old, plain, sometimes comic flavour.
The newer and now much more common one is in hospitals: vitals means vital signs — the basic body numbers that nurses check: temperature, pulse, breathing rate, blood pressure, and often oxygen level. 'Take his vitals.' 'Her vitals are stable.' 'Vitals are dropping.' If you watch medical dramas in English, this is the one you will hear every episode, and it is short for 'vital signs' rather than being about organs at all.
The word is always plural — there is no singular 'a vital' as a noun. And note that it is used with a plural verb: her vitals ARE stable.
