It is an internet word, not a dictionary-shelf word, and it belongs to a specific and harmful corner of the web: the pro-anorexia communities where people with eating disorders encourage each other's illness instead of getting help. The word is built to sound positive, like inspiration, and that is exactly the danger in it: it dresses up self-starvation as motivation. So the register matters. When doctors, teachers, journalists, or ordinary people use the word, they are almost always naming something as a problem — 'her feed was full of thinspiration' is a warning, not a description of a hobby. If you use it approvingly, English speakers will hear you as endorsing an eating disorder. It is not a word you need in daily conversation, but it is worth understanding, because it appears in news reports and in any serious talk about young people and social media.
