"Smeared" describes something spread across a surface in a messy way, not smooth or neat - like when wet paint or ink gets smudged across a page. If your hands are "smeared with" something, they're dirtied by it being wiped across your skin. People also use "smeared" for reputation - if someone is "smeared" in the news, false or unfair information was spread about them on purpose to make them look bad.
