A rude word for a boy or man who other boys think is weak, or who likes things they think only girls should like. It is an insult, and an old-fashioned kind of insult: it comes from the idea that being like a girl is something bad. That idea is the whole engine of the word, which is why many people now hear the word as saying something ugly about girls, not just about the boy it points at. Children use it on the playground; adults use it too, often about someone who is afraid of small things — cold water, a needle at the doctor, a scary film. Said between close friends who are joking, it can be light, almost : "Come on, don't be a sissy, the water's fine." Said seriously, it is meant to hurt, and it does. It also works as an adjective: a sissy voice, a sissy way of walking. Note that it is only used about boys and men. A girl is never called a sissy, however afraid she is. Some men have taken the word back and use it about themselves with pride, but that is a small, specific use; assume the ordinary use is .
組成sis (a short form of sister) + -y, the ending that makes a word sound small and childish, like doggy or Mummy. So the word literally says "little sister" — a boy treated as if he were his sister.
