A sicko is a person who enjoys things most people find disgusting or — cruelty, violence, or sexual things involving children or pain. It is a strong insult and always an insult; you call someone a sicko, you never call yourself one seriously. It is informal American-flavoured slang, common in speech, in comments online and in headlines, and out of place in an essay or a report. The feeling behind it is disgust plus fear, and people often say it about criminals in the news: "the sicko who did this". It can also be used as an adjective — sicko stuff, a sicko joke — though the noun is far more common. Two things worth knowing. First, it is much stronger than "weird": calling a a sicko is a real accusation, not teasing. Second, because it treats mental illness as a moral fault, many careful speakers avoid it, and it is never the right word for someone who is genuinely in the mind. Note also that it is not related to "sick" meaning excellent in young people's slang ("that's sick!" = that's great) — a sicko is never a compliment.
組成sick + -o. The ending -o makes a casual, often mocking noun for a kind of person: weirdo, wino, pinko. Here sick has its old figurative sense of 'mentally twisted, morally rotten', not 'ill with a fever'.
