Someone who is psychotic is very ill in the mind: they may hear or see things that are not there, hold beliefs that do not match the shared world, or lose the usual line between what is real and what is not. Doctors also use the word for things caused by that illness — a psychotic break, psychotic symptoms, psychotic depression. It can be a noun for the person, though that can sound cold, as if the illness were the whole person. In everyday talk people stretch it and call a wild plan or a very angry person psychotic. That use is loose and often unkind; it is not the medical meaning. Hospital and paper English keep it fairly formal. Though actually, it is not the same as psychopathic: a psychopathic person may hurt others and feel little guilt, but they are usually still in touch with what is real.
