You do something on the off-chance when you hope a thing will happen, even though you know it probably will not. You are not counting on it; you are only leaving a small door for luck. English speakers almost always say the whole phrase on the off-chance — you rarely hear off-chance by itself. It often comes with that: on the off-chance that she is still there. It is friendly, everyday English, a little old-fashioned in a kind way, in both British and American talk.
