A malted is a cold sweet drink, especially in the United States: milk, ice cream, and malt powder mixed together, often chocolate. People also call it a malted milk, or more loosely a malt. You order one in an old-style , not in a formal restaurant. When someone says “I’ll have a malted,” they mean that drink, not beer. The same form malted is also used before a noun for food made with malt (malted milk, malted ); that is the describing use, not “a malted.”
Malt is the special grain used for beer and whisky; here it is the malt powder in the drink.
