Something loathsome is so unpleasant that it makes you want to turn away — cruel, disgusting, or morally ugly. A loathsome crime, a loathsome smell, a loathsome character in a story. It can be about the body (filth, rot) or about the heart (cruel treatment), and English lets it cover both. It is a strong, slightly word; in everyday talk people more often say disgusting, horrible, or awful. When someone chooses loathsome, they usually want you to feel the old weight of inside it: this is not just “a bit nasty”. The -some here means “causing that feeling”, the same kind of ending as in burdensome or .
組成loath + -some. From Middle English lothsum, going back to an Old English shape *lāþsum, “causing loathing”. The ending -some means “having / causing that quality”. German leidsam is a cousin.
