Incoherent describes speech, writing, or thinking that does not hold together and makes no clear sense - the words might be there, but you can't follow the meaning. Someone who is extremely tired, drunk, in shock, or very upset might speak incoherently, jumping between ideas with no clear connection. It can also describe a plan or argument that has parts which don't logically fit together.
