    Hoarsely tells you how someone speaks when their voice is hoarse: in that low, rough way. It is simply the adverb for hoarse. 'He laughed hoarsely' means the laugh came out , not that he is a rough person. Same everyday causes as hoarse — a cold, too much shouting, a dry throat, a long cry. Neutral, ordinary English. You will meet it in stories more than on medicine boxes, because stories need to say how a line was spoken.
