The word is about effort and tiredness. It is not about blood or horror; that is gruesome, a different word. British English gruelling with two L's. American English usually with one L. The meaning does not change. It comes from an old verb to , 'to punish or wear someone out,' and that verb came from the thin food — the idea of being worn down as if you had been put on a hard, poor diet. Almost nobody uses the verb now. The adjective is the form you will meet.
