A gamer is a person seen through one activity: playing. Someone hands them a set of rules and a way to win, and they lean in. The word has stayed on that one picture for centuries while the board under it kept changing — dice, cards, a pitch, a screen, and finally the rules of ordinary life treated as if they were a game to be won.
From Middle English gamer, attested as a common noun before 1450; game (noun) + -er, the suffix that names the person who does something. The word has always meant simply 'one who plays games'.
