An extramarital sexual relationship is one that a married person has with someone who is not their husband or wife. Extra- here means outside; marital means to do with marriage. So it is sex outside the marriage. It is a fairly formal, slightly cold word — newspapers, lawyers, and doctors use it. Everyday speech more often says affair, or talks about cheating. It usually describes the relationship or the sex, not the person: you say an extramarital affair, not really an extramarital man. Though actually, the word is about marriage as a legal and social bond, not about whether the people are in love.
