An extern is a university student who goes to a workplace for a short time to try that job and see how it really runs — a hospital, a law office, a clinic. It is close to intern, and though actually even native speakers mix the two. The old difference sits in the Latin: intern points “inside” (the person has often joined the place, sometimes even lives there); extern points “outside” (the student still belongs to the school, comes in, then leaves). Today the neat line is messy. In a lot of places people just say intern for both, and “extern” survives mainly in a few training worlds, especially American medicine and law, with “” as the name of that short stay. It is not a dinner-table word. You may also meet an older, rarer use for a student who studies at a school but does not live there; the sense you need here is the short job for experience.
