Cortical means 'of the cortex' — of that outer layer of an organ, especially the brain. A cortical injury is damage in the outer brain layer, not deep in the middle. Doctors also talk about cortical bone: the hard, dense outer bone, as against the bone inside. It is a science and medical word; you will not hear it much in ordinary chat. If a word is cortical, it belongs to that outer layer, not to the core.
