To agonize is to turn a hard choice over and over in your mind until the thinking itself hurts. You weigh every side, you cannot decide, and you feel the struggle. People usually agonize over or about something: a job, a medical choice, whether to tell the truth. It is stronger than "think carefully"; there is pain in it. American spelling is agonize; British spelling is often agonise — same word, same sound. Though actually, older writing can use it for real bodily pain, like a person in agony, but the meaning you will meet now is almost always the mental one: a long, sore indecision, not a twisted ankle.
