If someone is with-it, they follow what is new and fashionable — in clothes, music, ideas, technology — and it shows. 'His grandmother is surprisingly with-it: she's on TikTok.'
Now the honest part. This is an old-fashioned way of saying 'fashionable', and that gives it a funny twist: today it is used mostly BY older people, or ABOUT older people, and often with a smile. A young person in 2026 will not say 'that jacket is very with-it'. So the word is a small trap — using it does not make you sound modern; it makes you sound like someone's uncle trying to sound modern. That is fine if you want the joke.
There is a second, different meaning that is still fully alive: 'with it' meaning awake, alert, thinking clearly. 'Sorry, I'm not really with it this morning — I only slept three hours.' 'Grandad's ninety-two and still completely with it.' In this sense it usually has no and comes after the verb.
That is the pattern to remember: and before a noun tends to be the fashionable sense (a with-it young manager); no and after the verb is usually the alert sense — though the fashionable one also appears after the verb, so context decides.

組成Just the ordinary words 'with' and 'it', frozen together into one describing word. The 'it' does not point at anything in particular — it means something like 'whatever is going on now'. English does this fairly often: think of 'in' meaning fashionable, or 'out of it' meaning dazed.
