組成well ('properly, competently') + run. This 'run' is not running with your legs — it is the 'run' of 'she runs a bakery', meaning to manage or be in charge of something. So 'well-run' = 'run well by someone'. That management sense of 'run' probably grew out of the idea of keeping a machine or a process going.
