Dirty in a way that could make people sick. This is not the word for a messy desk or a shirt with a stain — `unsanitary` is about hygiene, about and disease. A kitchen where raw meat sits next to cut fruit is unsanitary. A hospital that needles is unsanitary. Toilets with no running water are unsanitary. The word always implies a health risk, not just an unpleasant look.
Because of that, it belongs to official language: health inspectors, news reports, government notices, court cases. When a restaurant gets shut down in Taiwan, the English-language news will say it was closed for 'unsanitary conditions'. Ordinary people talking about their friend's messy apartment would say 'gross' or 'filthy', not 'unsanitary'.
You will also meet ``, which means exactly the same thing and is more common in British writing. Both are correct; American English strongly prefers 'unsanitary'.
組成un- (not) + sanitary (relating to keeping things clean for health). 'Sanitary' comes from Latin sanitas, health — the same root behind 'sane' and 'sanitation'.
