Not understanding what is going on. This word is almost always about the face, the eyes, or the look — an uncomprehending stare, an uncomprehending expression. That is its home. You use it when someone hears the words but the meaning does not reach them: a small child told that a has died, a patient with a failing memory, a person hearing terrible news in a language they barely speak. So it carries a feeling of gentleness or pity rather than blame. English writers reach for it in that frozen moment before a person takes something in. You will rarely hear it in speech; in conversation people say 'he didn't get it' or 'she just looked blank'. And it is not the same as 'stupid' — an uncomprehending face belongs to someone who is lost, not someone who is slow.
