Twirly is the adjective made from twirl. It describes something that has twists or curls in it — something whose shape goes round and round. Twirly handwriting has lots of loops. A twirly staircase winds up in a spiral. Twirly moustaches curl up at the ends. Twirly pasta is the kind shaped like little screws. It can also describe something that turns or is likely to turn: a twirly skirt is one that flies out nicely when you spin. The important thing about this word is its register. It is informal and slightly childlike — the sort of word you use with children, or when you are being light and casual and cannot be bothered to reach for the proper word. In careful writing you would say spiral, curling, ornate, or twisted instead. Nobody would use twirly in a report or a serious description; that is not a rule, it is just how it sounds. If a student writes "the twirly columns of the temple" in an essay, it will read as a bit sweet and out of place. But in speech it is friendly and perfectly natural: "I like the twirly bits on the gate." Because it is a casual word, it is also flexible; people make it up on the spot for anything curly. That is normal for -y adjectives in English.
