Rated at three stars — a mark of quality given by an official guide or rating system. It goes before a noun: a three-star hotel, a three-star restaurant.
How good three stars actually is depends entirely on the system, and this is where get confused. For hotels, the usual scale runs to five stars, so three-star means decent and comfortable but not luxury — clean, has what you need, mid-price. Fine, not fancy. For restaurants in the Michelin guide, however, the scale only goes to three, so a three-star restaurant is the very top: one of the best in the world, where people book months ahead and pay a great deal. Same words, opposite feeling. Always ask yourself which scale is being used.
It also appears for army rank in some countries — a three-star general — and on some product and hygiene ratings.
The whole set works the same way: one-star, two-star, four-star, five-star. Note that 'star' stays singular before the noun, and that five-star has drifted into everyday praise — 'five-star service' just means excellent, even with no official rating involved.
