A slip-up is a careless mistake — usually small, and usually because you were not paying attention, not because you cannot do the job. You sent the mail to the wrong person; you wrote the wrong date; you said the wrong name. It feels milder than a serious error or a crime: people use it to keep the blame light. “Sorry, that was a slip-up.” You will also hear the verb slip up (“I slipped up”). The noun is just that verb turned into a thing. Informal to neutral; fine at work in speech, a bit casual for a very formal report, where people may prefer mistake or error. Though actually some speakers write slipup as one word; the slip-up is the usual dictionary shape.
